Make sure patient has received comprehensive
medical examination to rule out any treatable disease as cause of
problems.

| 2. |
Prepare comprehensive list of all assets owned
by patient and spouse.
|
| 3. |
Gather together and organize financial
documents:

a. |
Bank
accounts owned by patient.
|
| |
b. |
Mutual funds owned by patient.
|
| |
c. |
Brokerage accounts owned by patient.
|
| |
d. |
Certificates of deposit in patient's name.
|
| |
e. |
Stock certificates registered in patient's name.
|
| |
f. |
Any
promissory notes under which patient is entitled to receive payment.
|
| |
g. |
Titles to motor vehicles and/or mobile homes that are listed in patient's
name.
|
| |
h. |
Deeds to real property owned by patient.
|
| |
i. |
Any
appraisals of jewelry or other valuable personal property owned by
patient.
|
| 4. |
Schedule appointment with Attorney to prepare
powers of attorney and Wills.
|
| 5. |
Prepare durable power of attorney for patient
and spouse.
|
| 6. |
Prepare medical power of attorney for patient
and spouse.
|
| 7. |
Prepare Living Will for patient and spouse.
|
| 8. |
Prepare Last Will and Testament for patient
and spouse.
|
| 9. |
Schedule appointment with insurance agent to
determine whether changes in insurance are desirable.

| 10. |
Review and consider insurance coverage for:

Life
insurance. |

(2) |
Health insurance. |
| |
(3) |
Long term care insurance. |
| |
(4) |
Homeowner's insurance. |
| |
(5) |
"Umbrella" insurance. |

Consider whether qualification for Arizona Health Care Cost Containment
System ("AHCCCS") is desirable.

Consider whether qualification for Arizona Long Term Care System ("ALTCS")
is desirable.

Determine how to best protect patient from exposure to predators.
|
| 14. |
Make
contingency plans so primary care giver obtains respite and to provide for
secondary/alternative care giver.

Consult attorney to determine whether asset protection planning and/or
exemption planning are advisable.
